Community First Services & Supports (CFSS)

What is CFSS?

Community First Services and Supports (CFSS) is a Minnesota health care program that helps people with disabilities, chronic illnesses, and older adults live independently.

Every CFSS participant is required to pick between two CFSS service models - the Agency Model or the Budget Model. The model you choose is up to you and depends on how much control and flexibility you want.

**On October 1, 2024, CFSS officially replaced two older programs: Personal Care Assistance (PCA) and the Consumer Support Grant (CSG). If you were on either of those programs, you are required to switch to CFSS. You will need a Service Delivery Plan, and we will help with that.

Who is Eligible for CFSS?

• You may be eligible for CFSS if you are enrolled in Medical Assistance (MA) or certain waiver programs.

• If you need assistance with at least one activity of daily living (ADL) like dressing, grooming, bathing, eating, toileting, walking, transferring or bed mobility or have Level I behavior needs.

• You have completed a MnChoices Assessment or are scheduled for one.

*Final eligibility is determined by your Care Coordinator/Case Manager or MnChoices Assessor.

Activities of Daily Living (ADLs):

• Walking

• Dressing

• Transferring

• Bed mobility

• Eating

• Toileting

• Bathing

• Grooming

Instrumental Activities of Daily Living: (IADLs):

• Meal preparation

• Grocery shopping

• Medication reminders

• Light housekeeping

• Laundry

• Money management

• Help with paperwork

• Transportation to appointments
