Community First Services & Supports (CFSS) Process Timeline
Step 1: MnCHOICES Assessment
Participant completes a MnCHOICES Assessment with their county/case manager, Tribal Nation, or Managed Care Organization (MCO) to determine CFSS eligibility.
Step 2: Referral to Consultation Services
A referral is made to a Consultation Services Provider, such as Altruest Services, if the participant meets eligibility for CFSS.
Step 4: Provider Attachment in MnCHOICES
The county/case manager, Tribal Nation, or MCO attaches Altruest Services in MnCHOICES as a provider, so assessment documents can be accessed to help develop the CFSS Service Delivery Plan (SDP) / DHS 6893P.
Step 3: Service Agreement Authorization
The county/case manager, Tribal Nation, or MCO submits a service agreement for Consultation Services so services can begin.
Step 5: Intake Scheduling
Once the service agreement is received, Altruest Services contacts the participant and/or representative to schedule intake.
Step 6: Development of the CFSS SDP
Altruest Services works with the participant and/or representative to complete the required CFSS Service Delivery Plan (SDP) / DHS 6893P. Education is provided regarding CFSS services, Budget vs. Agency models, participant rights and responsibilities, and informed decision-making.
Step 9: Final Approval & Ongoing Updates
The county/case manager, Tribal Nation, or MCO reviews the plan and works with the participant/representative and Altruest Services to approve the plan or request corrections if needed. Updates and revisions are completed throughout the year as necessary.
Step 8: Plan Review & Upload
Altruest Services reviews and approves the completed plan, uploads it into MnCHOICES, and notifies the county/case manager, Tribal Nation, or MCO that the plan is complete.
Step 7: Signature Collection
Required signatures are obtained for the finalized CFSS SDP / DHS 6893P.
