We are passionate in helping individuals and families navigate CFSS, make informed choices, and live independently at home.

We value clear communication. We will work with you, your representative, your family, caregivers, care managers and CFSS Agency or FMS Provider to ensure a smooth outcome.

Person-Centered Support: We help you develop your CFSS Service Delivery Plan (SDP) and provide ongoing information, training, quality improvement guidance and year-round support.

We understand the complexities of the CFSS Program. We have reviewed hundreds of CFSS Service Delivery Plans since CFSS launched on 10/1/24.

We proudly support individuals of all backgrounds, cultures, identities, and abilities. We are committed to fostering an inclusive, respectful, and welcoming environment while continually learning and strengthening our cultural awareness and competency.

Altruest Services LLC is owned and operated by two Minnesota-licensed social workers with almost 20 years of combined experience in care coordination, waiver case management, PCA, and CFSS services.

Throughout our careers, we have had the privilege of supporting individuals with diverse abilities, backgrounds, cultures, and languages. We have always found great purpose in this work and are passionate about helping people live as independently and confidently as possible.

We started Altruest Services because we believe participants deserve to fully understand their options and feel empowered to make informed decisions about their services. Our goal is to provide personalized, compassionate guidance and help make the CFSS process easier to understand, navigate, and implement.
